Mariposa Phlebotomy — Mobile Phlebotomy Services

These Terms and Conditions ("Terms") govern your use of the services provided by Mariposa Phlebotomy ("we," "us," or "our"), a mobile phlebotomy service operating in California. By scheduling an appointment, contacting us, or using our services in any way, you agree to be bound by these Terms in full. Please read them carefully before booking.

1. Services

Mariposa Phlebotomy is a mobile phlebotomy service. We travel directly to our patients to perform professional blood draws and specimen collection. All services are performed by trained and certified phlebotomy professionals.

Most laboratory tests require a valid physician's order. Some self-pay wellness panels may be available without a physician's order — please contact us in advance to confirm. We reserve the right to decline service if a required physician's order is not provided at the time of the appointment.

2. Appointments and Scheduling

Appointments can be scheduled by phone, online at www.mariposaphlebotomy.com, or through our AI phone receptionist. When booking an appointment, you agree to:

  • Provide accurate personal and contact information

  • Have your physician's lab order available at the time of your appointment

  • Have a valid photo ID ready for verification

  • Provide a safe, accessible location for the mobile visit

  • Follow any preparation instructions provided at the time of booking (such as fasting requirements)

  • Ensure that a parent or legal guardian is present if the patient is a minor

We will do our best to arrive within the scheduled time window. Travel times and traffic conditions may occasionally affect arrival. We will notify you as soon as possible if we anticipate a delay.

3. Cancellation and Rescheduling Policy

We require a minimum of 48 hours' notice to cancel or reschedule an appointment. This allows us to offer your time slot to another patient in need.

Cancellations made with less than 48 hours' notice or failure to be available at the scheduled location at the time of the appointment (no-show) may result in:

  • A cancellation or no-show fee

  • A deposit requirement for future bookings

4. Payment and Billing

Payment is due at the time of service. We accept the following payment methods:

  • Cash

  • Credit and debit cards

  • Zelle

  • Insurance (where applicable — see below)

For patients using health insurance, you are responsible for any co-pays, deductibles, or amounts not covered by your plan. Please verify your coverage with your insurance provider prior to your appointment. We are not responsible for claim denials or coverage decisions made by your insurance carrier.

Superbills: Mariposa Phlebotomy provides superbills upon request. A superbill is an itemized receipt containing CPT and ICD-10 codes that you may submit directly to your insurance company for out-of-network reimbursement. Please request your superbill at the time of your visit or contact us after your appointment. Superbills are provided within 1 to 3 business days.

5. SMS Messaging Terms

Program Name: Mariposa Phlebotomy Appointment Notifications

Program Description: We send SMS text messages to confirm appointments, send reminders, and follow up regarding your mobile phlebotomy visit.

Message Frequency: Message frequency varies based on appointment activity, typically 1 to 3 messages per appointment.

Message and Data Rates: Message and data rates may apply depending on your mobile carrier plan.

To Opt Out: Reply STOP to any message at any time. You will receive one final confirmation and will not be contacted further via SMS.

6. AI Phone Receptionist

Mariposa Phlebotomy uses an AI-powered phone receptionist to answer calls, assist with appointment scheduling, and respond to general inquiries 24 hours a day. This system is designed to provide accurate, helpful, and professional service.

If at any time you prefer to speak with a human staff member, simply say "speak to someone" or "transfer me" and you will be connected to our team or offered a callback.

Calls may be recorded and transcribed for quality assurance and record-keeping purposes. By calling us, you consent to this recording.

7. Lab Results and Medical Disclaimer

Mariposa Phlebotomy is a specimen collection service only. We do not interpret, diagnose, treat, or provide medical advice based on lab results. All results are forwarded to your ordering physician. Please follow up directly with your doctor for clinical interpretation, diagnosis, or any recommended next steps.

In the event of a medical emergency, please call 911 immediately. Our services are not a substitute for emergency medical care.

8. Patient Responsibilities

To ensure a safe and successful mobile visit, patients agree to:

  • Disclose any known bleeding disorders, needle phobias, or relevant medical conditions prior to the visit

  • Drink adequate water before the appointment to assist with specimen collection

  • Provide a clean, safe, and accessible environment for the phlebotomist

  • Inform us immediately if they feel unwell during or after the procedure

9. Limitation of Liability

Mariposa Phlebotomy is not liable for delays or errors caused by third-party laboratory processing, insurance carrier decisions, or telecommunications disruptions. We are not responsible for outcomes resulting from inaccurate information provided by the patient at the time of booking or service.

Our liability for any claim arising from services rendered is limited to the amount paid by the patient for that specific service.

10. Privacy

Your privacy matters to us. The collection and use of your personal and health information is governed by our Privacy Policy, available at www.mariposaphlebotomy.com/privacy-policy. By using our services, you agree to the terms of our Privacy Policy.

11. Governing Law

These Terms and Conditions are governed by the laws of the State of California. Any disputes arising from or related to these Terms or our services shall be resolved in the courts of California.

12. Changes to These Terms

We reserve the right to update these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Continued use of our services after changes are posted constitutes your acceptance of the revised Terms.
